How can I obtain my birth certificate by mail?

Write or call the Department of Vital Statistics at City Hall, Room 154, 165 Church Street, New Haven, CT 06510; the telephone numbers are 946-8084 and 946-7931. The following are required:
1. A copy of a valid Driver's License or Passport
2. $5 for a wallet size or $ 10 for full size  as a money order made payable to Treasurer City of New Haven
3. The applicant's name, date of birth, parents' names and mother's maiden name
4. A return address and daytime telephone number so that the department can ask the applicant any questions or clarifications
5. An indication of whether a full size or wallet size copy is being requested
Expect a 2-8 week turnaround period for requests to be processed
