How do I get a certified copy of my birth certificate?

A certified copy of a birth certificate is obtainable at the Department of Vital Statistics, City Hall, 165 Church Street, telephone numbers 946-8084 and 946-7931. The birth had to have taken place in New Haven.
The following have to be given: the date of birth, the parents' names, and the mother's maiden name
A valid Driver's License or Passport is required. If a picture ID is lacking, then any two of the following will suffice:
1. Social Security card
2. Written verification of identity from employer
3. Automobile registration
4. Copy of a utility bill
5. Checking account deposit slip
6. Voter registration card
The cost is $5 for a wallet size or $10 for full size copy, per copy. They can only be obtained by the individual if over 18, otherwise by a parent or legal guardian with proof.
Payment cannot be made by check, but only in cash or by a money order payable to Treasurer City of New Haven.
